Patient Forms

If you are a new patient you will be given instructions on how to set up a secure patient portal where you will complete new patient paperwork and clinical history forms. By completing the paperwork in this way your information will stay secure and will be placed electronically into your medical record. Your patient portal will be your primary means of communicating with your provider outside of your scheduled appointment times unless you prefer to communicate with the office by phone. If you prefer not to, or are unable to, complete the new patient paperwork electronically, you may have them mailed, faxed, or emailed to you for you to print and complete the paperwork. Regardless of which way you choose to complete your new patient paperwork and clinical history forms they must all be completed and signed by you before you can be seen by your provider. This ensures that you and your provider can make the best use of time during this important intake visit to address what is most concerning to you. If you require assistance in completing any of the paperwork we will be happy assist you upon request.
